General Decline Patient

The Omega Health Care team are experts in treating patients with no specific diagnosis however experiencing a rapid decline in health and needing assistance staying in their home.   

Consistent care of our patients allows them to experience comfort and the best quality of life possible.   We develop individualized plans of care.  As the disease progresses we design plans that address common problems associated with General Decline:

  • weakness
  • recurrent infections
  • weight loss
  • nutrition
  • hydration
  • pain
  • decreased mobility
  • decline in ability to do ADLs
  • skin care
  • psychological needs
Download General Decline FlierWe care for patients wherever they call home whether in their own home, a caregiver's home, a long term care facility or an assisted living community.
 
We will coordinate the individualized plan of care with the advice and consent of the patient's physician. The case manager will ensure that information flows between all physicians, nurses, social workers, aides, volunteers, and, if appropriate, clergy.
 
We will supply all medications, medical supplies and medical equipment related to the diagnosis to ensure patients have everything they need.
 
We will support the patient as well as the family emotionally and spiritually providing the resources to help both maintain their emotional and spiritual well-being.
 
We will train the caregiver on how to provide basic care to ensure the patient is comfortable and safe in the home. As the patient gets weaker, symptoms increase and communication becomes more difficult, we educate on how to best continue care.

Criteria for Admission

Hospice Criteria for Patients with General Decline 
 
Factors 1, 2 and 3 are more predictive of a poor prognosis with 4-9 providing further support of the prognosis.
  1. Worsening of Clinical Status
  2. Worsening of Symptoms
  3. Worsening of Signs
  4. Labs, when available (lab testing is not a requirement)
  5. Decline in Karnofsky Performance Scale (KPS) or Palliative Performance Scale (PPS) from <70% due to progression of disease
  6. Decline in Functional Assessment (FAST) for dementia (from > or = to 7A on the FAST)
  7. Progressing dependence for performance of ADLs
  8. Progressive stage 3 or 4 pressure ulcers, despite excellent wound care
  9. Increased ER visits, hospitalizations, or physician visits related to the hospice primary diagnosis
*If the patient doesn't meet criteria for a specific diagnosis guideline, a "Non Disease Specific Baseline combined with Disease Specific" guideline can be used.
